- An NDIS provider takes payment for services they do not provide by falsely instigating that they do.
- An NDIS support worker fails to observe the standard NDIS safety and quality guidelines, either knowingly or due to negligence, which creates a situation that could potentially harm the NDIS participant or potentially harm the NDIS participant.
- An NDIS provider takes above-average charges for their services from NDIS participants.
- An NDIS provider support worker or plan coordinator provides false or misleading information to an NDIS participant or their family member.
- Acts amount to criminal activity.
How to Report NDIS Fraud?

- The most feasible option is to fill out the online tip-off form (https://www.ndis.gov.au/form/ndia-fraud-reporting-form).
- You can also call 1800 650 717 (NDIS Fraud Reporting and Scams Helpline). The Helpline is available from Monday to Friday (9 am—5 pm AEST).
- Furthermore, the following individuals can lodge an official complaint on your behalf:

- You have enough time. Remember that the form must be completed in one session; you cannot save it for later.
- Have a trusted person nearby while filling out the form if you need help.
